Hospital in the Home is hospital-level care in the comfort of your aged care home. Hospital in the Home is free, hospital-level care in your own home, eliminating the need for an inpatient stay.

Receiving care at home helps reduce the risk of hospital-acquired infections and complications. You can spend more time with family and loved ones, and recover better in a familiar environment. 

GEM At Home

If you’re an older adult living at home and recovering from an illness or injury, GEM At Home can help you.

GEM At Home is a free specialised rehabilitation service that aims to maximise your recovery and function while reducing hospital time.

Hospital care in the comfort
of your aged care residence

Residential In Reach

Residential In Reach brings free specialised clinical care right to your residential aged care home.

Our goal is to enhance your comfort and well-being without the need for travel to hospitals or emergency departments.

This short-term service works alongside your regular care team so it’s not a replacement for the care you receive from your aged care home or your usual GP.

At Home or After
Hospital Care

Post-Acute Care

If you’ve recently been discharged from hospital and need short-term care at home to help you recover, Post-Acute Care can help.
This free service offers a personalised care plan to meet your individual and clinical needs. It will also arrange the services you need to help you recover.

District Nursing

This service provides advice, education and support to you at home. It is a nursing service delivered to you at home and is designed to encourage self-management and independence.
Our focus is helping maintain independence while delivering the best clinical care to suit your needs.

Complex Care/HARP

Complex Care provides support to people living with chronic disease, those who are older and/or have complex needs.
Complex needs might include conditions like psychiatric disability or mental health issues, or people who frequently use hospitals or are at risk of unplanned hospitalisations.

Home Care Packages

If you’re aged 65+, a Home Care Package, sometimes called an aged care package, may provide the help you need.

The ultimate goal of a Home Care Package is to help you or your loved one stay independent at home, and continue doing what makes you, you!
