Every person in Victoria should be able to access the right care, in the right place, at the right time – wherever they live.
To support this, Local Health Service Networks (Networks) across Victoria will be formed from July 2025.
These Networks (made up of hospital services) will be responsible for supporting coordinated care to the Victorian community. For more information on the Networks visit health.vic.gov.au/local-health-service-networks
To help inform how hospitals work together to provide you care as a Network, we want to hear about how you access care.
Your experiences are valuable and will remain anonymous.
It is important that we are hearing about your experiences over the last three years in accessing healthcare. This will help ensure that the information provided to the Networks addresses the current opportunities and needs of Victorians.
How to participate
This survey is seeking insights from community members and the healthcare workforce. It will take about 10 minutes to complete. The survey will close 8 June 2025 at 11.59pm.
