Hospital care in the comfort of your home

Did you know Dhelkaya Health has a dedicated team of nurses and allied health clinicians that provide hospital equivalent care in the comfort of your own home?

Our ‘At Home’ programs offer free, hospital-level care in your own home, eliminating the need for a hospital stay. Receiving care at home helps reduce the risk of hospital-acquired infections and complications. You can spend more time with family and loved ones, and recover in the familiar comfort of home.

Our programs include:

  • Hospital in the Home
  • Geriatric Evaluation and Management (GEM) At Home (short-term treatment at home for older people with age-related conditions)
  • Residential In Reach (free specialised clinical care to your residential aged care facility)
  • Rehabilitation in the Home (RITH) (coming soon).

To be eligible for Hospital in the Home, you will must live within the Mount Alexander Shire, have a local GP with admitting rights to Dhelkaya Health and present with a medically stable acute condition. You can be admitted directly by your GP, urgent care centre, acute ward or another health service. You’ll need access to a carer or support person and will benefit from daily visits from the At Home team of clinicians. 

Once admitted, you’ll get details about your condition and treatment plan. Registered nurses will visit you as needed, and allied health professionals (like physiotherapists or dietitians) may also provide services.  Regular check-ups with your GP will occur at your home or at your GP’s clinic or Dhelkaya Health. Even while at home, you remain a registered inpatient of Dhelkaya Health.

Feedback from members of the local community who have received care from the At Home team has been overwhelmingly positive. Many described the visiting staff as “friendly, reassuring, and like part of the family.” One person shared, “They explained the care I needed with antibiotics and changed my wound dressings, always reassuring me that I was improving during their daily visits.”

The At Home nurses also spoke about the impact of their work, saying “We love being able to offer hospital-equivalent care in someone’s own home. It’s incredibly rewarding to see someone improve and regain confidence throughout their home-based admission.” Another nurse added “No two days are the same. We really value the diversity of care and treatment we provide in the home environment. We work closely as a team with the patient’s GP, social workers, occupational therapists and physiotherapists to provide truly integrated care.”
